INFORMATION FOR ANNUAL MEETING (Youth and Adult Chaperones/Pastors)

The Medical Authorizations are for you and to keep with your group.  The Illinois Conference needs the Covenant Forms (both Youth and Adult) and the Safe Church Disclosure Form.  Please turn these forms in to Rev. Paula Comper at the Conference Office.  Thank you for your participation.
